Airedale’s Digital Care Hub is the culmination of over a decade of cutting edge development work in the fields of telemedicine and digital health.
The Hub currently provides round-the-clock care support to 500 care homes nationwide, over 2000 patients in their last year of life, more than 30 prisons, and thousands of patients with long-term conditions living in their own homes.
The Hub is now providing clinical support to 123 care homes in the Bradford, Airedale, Wharfedale and Craven district during the coronavirus outbreak. The service has been enhanced with the addition of GPs, GPs with special interests, hospital consultants, pharmacists, mental health specialists and social care to the already established base of nurses.
Development of the enhanced service has been carried out at pace, with the new arrangements having been put in place in just three weeks.
The service:
supports people to remain well in their own home, providing expert clinical care from a range of specialists
offers support and guidance to care home staff on the care of residents
reduces the risk of coronavirus transmission by providing video consultations
supports people to be safely admitted to hospital, when that’s the best place to meet their health needs.
offers specialist mental health support around supporting well-being
aims to introduce Allied Health Professional (AHP) therapy staff to support rehabilitation and therapy through video consultation
